I am ___________, and my greetings to you. I am present today to come up with an interdisciplinary plan that would deal with major challenges in Riverside Medical Center. This plan is expected to increase care coordination since it is a current factor that causes confusion and ineffective communication between departments when they change shifts. As a charge nurse, I ensure patient needs are addressed, management of nurse assignments is addressed, and inter-department communication is effective. The idea focuses on enabling the providers to enhance their collaboration, better patient outcomes, and nurse retention that enhances the efficiency of the healthcare system (Paguio et al., 2020).

Slide 2

The communication plan aims at achieving the goal of enhancing cooperation between administration and nursing at Riverside Medical Center through the application of structured tools and strategies to strengthen the collaboration between the two in the process of shift changes. The main goal is to enhance patient care by simplifying the process of exchanging information, processing the workloads more effectively, and making better decisions. The protocols are formulated after involving all the organization members to ensure that they are not left out. This strategy is aimed at making sure that the needs of the patients and medical personnel are taken into account and that all parties are transparent and responsible (Brenner et al., 2022). The charge nurse also says that a failure to communicate between healthcare providers may lead to issues with patient care when transferring care between shifts.

Slide 3

Organizational Issue

Riverside Medical Center has a high need to upgrade the communication departments because the shift change may result in slowing patient service and the emergence of safety challenges. Providing significant information to administrative personnel about patients is at times a challenge to the nurses, which may influence the quality of decision-making and operations within the hospital. To address these issues, nurses, administrators, IT experts, and patient advocates will collaborate in an effort to establish and adhere to adequate healthcare communications guidelines. The use of this new approach will result in improved treatment decisions and care quality of the patients (Keskinocak & Savva, 2019). The charge nurse noted that patient care is also hard to offer at all times due to the lack of coordination among healthcare providers.

Effective communication of nursing and administrative staff during shift changes in Riverside Medical Center is important to provide good treatment plans, sustained care, and timely response to patients. With such improvements, the hospital will manage to provide patient-centered care that will lead to improved outcomes and quality of care among patients (None et al., 2024). The recommended protocols will enable the flow of patient data between departments to be easily conducted.

Slide 4

Consequences of Not Addressing the Issue

Poor communication will most likely result in disjointed coordination in patient care at Riverside Medical Center once the shifts have shifted. Nurses struggle to report on critical patient issues to the administrative personnel, hence causing delays in administration and treatment delivery as well as decision-making. In case of a communication break, the work of the hospital is damaged, the threat of medical errors and patient exposure increases (Yaacoub et al., 2020). Addressing such issues will lead to improved hospital functions and improved patient care.

Lack of communication between the departments at Riverside Medical Center will impact the patients. The inability to address patient needs promptly due to the lack of effective communication may lead to misunderstandings, lead to the extension of the response time, and, in certain cases, endanger patients (Pryce et al., 2021). The failure of communication will deteriorate the quality of service, which results in issues of delivering efficient and timely services and may jeopardize the recovery of patients. The solution to these issues will lead to the improvement of patient care and effective healthcare.

Relevance of Interdisciplinary Team Approach

In the case of Riverside Medical Center, an interdisciplinary team approach plays a vital role in enhancing communication between departments during shift changes since it involves the integration of many different views towards resolving the problem of lack of coherence in the coordination of care. The team comprises nurses, IT experts, administrators, and patient advocates. Nurses give firsthand experiences of the patients served, administrators see the holistic picture, and patient advocates remind them of what is in the best interest of the patients (Bhati et al., 2023). IT professionals are important to ensure the smooth use of relevant communication technologies to streamline processes. Effective collaboration between these teams will enable them to record improved patient outcomes and ensure that work procedures are organized.

Slide 6

Evidence-Based Interdisciplinary Plan Summary

Objective

The essence of this interdisciplinary plan is to establish effective communication between the departments in case of shift changes at Riverside Medical Center. The primary objective is to connect an electronic handoff tool to the electronic health record (EHR) system that will facilitate the transfer and the other departments in organising patient information more easily. This is to prevent mistakes that can jeopardize the lives of patients. The use of the EHR extension will be facilitated by having regular interdisciplinary conversations and applying communication guidelines daily (Engle et al., 2021). It is directed to increase accountability of the healthcare process, improve teamwork work and result in smoother coordinated care that makes the system and patients operate efficiently.

Working

The communication enhancement strategy employs a model of continuous improvement, which is now focused on the implementation of the electronic handoff tool. The team will start by defining areas of key communication differences when the department is changing, and its members will set particular objectives to achieve. Once they have changed EHR, they evaluate its impact on collaboration and safety through data collection (Verma et al., 2021). Through these evaluations, the best practices are modified and standardized, including the inclusion of correct and timely upgrades and adequate handover documents. The ability to learn in other areas and to make changes will foster a culture where individuals continuously make improvements that will lessen any problems in communication and benefit patients and employees.

Role of Interdisciplinary Team

The staff of Riverside Medical Center consists of specialists in different spheres. The group of nurses, administrators, IT experts, and patient advocates employs their various experiences to enhance the coordination of care. When administrators manage resources and plan the goals of the organization, nurses observe and solve the discrepancies in the way people communicate that influence patient care. The patient advocates ensure the incorporation of what the patients view in the communication process, and the IT specialists simplify the sharing of information by means of technology (Tamli & Sain, 2023). The collaborative work team assists the departments in applying effective communication techniques to ensure that they have accurate and timely exchanges. The hospital conducts regular conferences concerning the protocols and gathering the ideas of their employees, so the staff members can optimize the way they treat patients and handle their duties.

Slide 7

Implementation of Interdisciplinary Plan

The interdisciplinary strategy at Riverside Medical Center will be applied in a step-by-step manner to make the most out of it. It will create a team consisting of nurses, administrators, IT specialists, and patient advocates whose duties will be specific (Abbasinia et al., 2020). Regular meetings will be conducted to go through the communication plan, engage every department, and update it where needed so that it can perform its role in the day-to-day activities effectively. IT experts will educate workers on the new communication tools and assist in their usage. Evaluation and monitoring activities will be conducted frequently during the implementation to ensure the process and implement changes (Errida & Lotfi, 2021). By working together, healthcare providers concentrate on the process of enhancing communication and collaboration among all parties involved in the effort to produce the most favorable outcomes for patients.

The degree of staff satisfaction, patient safety, and effectiveness of communication will be evaluated regularly so that the success of the plan and opportunities to address improvement areas can be judged. The team will continue to offer feedback to one another to improve the strategies of communication and solve all problems. This will be done with the help of administrators, who will direct the work, address the resistance, and ensure the plan continues its dynamics (Okolo et al., 2024). The interdisciplinary approach allows the team to invite the appropriate parties to participate in a systematic process that assists them to interact and cooperating more effectively, yielding positive results for Riverside Medical Centre.

Slide 8

Management of Human and Financial Resources

An interdisciplinary communication plan will only be successful when Riverside Medical Center manages its people effectively and its finances. The key points of the plan will be supported by a proposed budget of 300,000. The budget will include the cost of recruiting the right individuals, which is 30,000, the cost of other training programs will be 50,000, the incentives like bonuses and development plans will be 120,000, and another 100,000 on technology like modern patient care systems and communication tools to assist in efficiency. The budget can be used to manage resources effectively and ensure the successful implementation of the change plan, which will aid in enhancing collaboration, making work easier, and achieving better patient care outcomes (Ștefan et al., 2024).

Similarly, Riverside Medical Center has implemented similar programs to assist in communication and improve the organization of the transition between one shifts. The communication technology has also been advanced, including the use of HER system and secure messaging, to enhance the manner in which various healthcare teams collaborate. The hospital has invested in the development and training of its staff, which has contributed to the decrease in the number of staff who are leaving and the improvement in how they perform their work (Singh et al., 2024). The positive outcomes of enhanced teamwork, enhanced decision-making, and enhanced patient care are evident, and hence the need to invest in technology and staff to have an effective healthcare environment.

Slide 9

Criteria to Evaluate the Project Success

There are some evidence-based criteria that will be used at Riverside Medical Center to assess the success of the communication improvement plan. The first surveys will be distributed to the nursing and administrative staff to understand whether the new means of communication are reflecting their expectations. In so doing, the two departments will be aware that they are supported and that they have all the resources necessary to deliver excellent care to the patients. Moreover, the patient safety procedures, such as preventing medication errors, improving responsiveness and responsiveness and offering prompt care to patients in critical condition, should be monitored to measure the role of improved communication in enhancing patient outcomes (Mostafa and El-Atawi, 2024). Through these measurements, professionals will be able to steer more improvements to the communication process to ensure the outcomes of good care are maintained.

The effect of the communication improvement plan will also be examined at Riverside Medical Center through the examination of the efficiencies that have been enhanced within the facility. This is the knowledge of the amount of time devoted to patient care, how fast the staff members can accomplish their tasks, the ease of changing a shift, and enhancing the interaction between different departments. Moreover, the hospital will monitor adherence to medication among patients, as well as the reduction of readmission, to determine whether the changes are enhancing the patient outcomes (Cram et al., 2022). The established criteria will confirm the beneficial changes in patient care, the happiness of the staff, and the management of the hospital.

Slide 10

Conclusion

The solution to the communication challenges between departments in the event of handing it over is vital in enhancing patient care, operational efficiency, and high levels of employee satisfaction at Riverside Medical Center. The obstacles to effective communication can be overcome by adopting an interdisciplinary approach, which promotes collaboration, creates communication protocols, and introduces specific incentives. The plan will help to increase patient safety, better clinical outcomes, and cohesion of staff, as well as make them more accountable by developing staff, investing in technology, and providing feedback more often.
