-Complete a comprehensive plan to reduce fall risk for patient as a team.​

-Assess , educate and provide care for patient as a team.  

PLAN DO STUDY ACT
PLAN: Use Interdisciplinary approach to prevent fall risk. Doctor, Pharmacist, Nurse, and Physical Therapist will be involved as a team in assessing patient when admitted to hospital, providing thorough assessment of history of falls as well as cause and factors to fall occurrence.

DO:  Interdisciplinary team will meet weekly or as needed to debrief about fall risk patients after comprehensive routine assessments. Each team member will play their designated role as well as transfer knowledge and share expertise to achieve a collaborative patient goal. NURSE: educate patient, set fall precations, conduct environmental assessment, respond to alarms  
DOCTOR: assess for fall precations, initiate fall precations, educate patient, assess medication
PHYSICAL THERAPIST:assess for fall precations, educate patient about fall precations
PHARMACIST:assess medication record, share expertise with interdisciplinary team.

STUDY: 
Nurse administrator will evaluate fall rate prior and after proposal plan.
Conduct interdisciplinary feedback 
Evaluate new behavior as team collaboration.  

IMPLEMENTATION AND RESOURCE MANAGEMENT

Evidenced based practice will be implemented as a resource. 

The Fishbone diagram will be introduced as a source of analyzing the cause and effect for falls in the rehab center.

EVALUATED GOALS:
MINIMIZED FALL RISK BY 65% AND REDUCED HOSPITAL EXPENSES CAUSED FROM FALL RISKS ​
IMPROVED WORK ENVIROMENT AS WELL AS DECREASED # OF FALLS WITHIN THE YEAR.​

HOW WILL WE MEASURE THIS OUTCOME?  ​ FALL RATE EQUATION (ABOVE) WILL BE USED TO EVALUATE FALLS PER MONTH.​ ​  ​ ​
