PLAN: Use Interdisciplinary approach to prevent fall risk. Doctor, Pharmacist, Nurse, and Physical Therapist will be involved as a team in assessing patient when admitted to hospital, providing thorough assessment of history of falls as well as cause and factors to fall occurrence.

DO: Interdisciplinary team will meet weekly or as needed to debrief about fall risk patients after comprehensive routine assessments.
