Strategy for multidisciplinary treatment under the direction of caregivers, with nursing staff at the forefront of the procedure, communication, and decision-making (Konradsen et al., 2022), strengthening patients and families by involving them in decisions, interactions, and care (Dijkstra et al., 2022). However, to enhance the communication with the patient, assistance should be given in a variety of languages, comprehensive treatment based on cultural norms should be provided, and respect and empathy should be shown (Konradsen et al., 2022), implementing a paradigm of collaborative decision-making and exchanging information with coworkers and colleagues to facilitate informed decision-making, straightforward communication is preferred to that it’s indirect. For example, throughout diagnosis, open communication is useful, but indirect communication may lead to disagreements (Konradsen et al., 2022).Furthermore, the use of effective communication throughout conversations and decision-making encourages better therapeutic selection decision-making, as opposed to the use of persuasive communication (Courtemanche et al., 2018), caregivers, patients, doctors, and other stakeholders may enhance coordination by incorporating patients at the system, individual, and organizational levels since everyone is engaged in direct communication, debate, and decision-making. Additionally, Reducing the use of chat apps and boosting the use of talks, monitoring systems like medication administration errors, and the licensing, prescribing, and management of medicine all increase patient safety (Courtemanche et al., 2018), enhancing collaboration and information sharing between HCPs by using the EBP model to change implementation and outcomes evaluation, effective problem-solving and team management depend on the use of root-cause analysis and dispute resolution tactics (Courtemanche et al., 2018) and interpersonal support and training on how to offer holistic, intellectual, emotional, and moral assistance are essential since they create a safer working environment. This promotes more quality of healthcare (Dijkstra et al., 2022). You exhibit a high level of accountability and openness by disclosing mistakes right away and abiding by medical and government standards (Dijkstra et al., 2022).

Difference between Patient Experience and Satisfaction

Here’s an illustration: Suppose that a 62-year-old female and a 21-year-old male patient in the same hospital system got the same treatment. The fourth factor is the quality of training and proficiency of the caregivers and HCPs; the better the outcome, the more competent they are in patient-centered care, culture-based care, nurse-patient interactions, patient security and stability, and patient and relatives administration (Dijkstra et al., 2022).The fifth factor is offering care in a range of languages since communities sometimes contain people from different cultures. Lack of multilingual assistance lowers positive patient outcomes and makes it more challenging for patients to increase their quality of healthcare. The nurse-to-patient ratio is the sixth aspect to take into account; the smaller the ratio, the higher the chance of burnout and mistakes. Errors in medication, administration, and record-keeping all have an impact on the standard of treatment and the reputation of medical facilities.

Rationale for Ethical Decision Making in Coordinated Care Plan

Although it covers key ethical concepts including equity, tolerance, fairness, transparency, integrity, confidence, accessibility, and compassion, as well as accountability, the integrated care plan is built on ethical decision-making. The utilization of patient-centered and culturally appropriate care, whereby each patient is addressed according to their traits and attributes, addresses the justice component (Haahr et al., 2020). Direct communication is used to address justice, and all patients are welcomed with empathy, engagement, and assistance. It thus enhances the standard of treatment and care quality (Haahr et al., 2020). As cost-effective treatment is a key component of the integrated care plan, which guarantees that everyone has access to basic services and its resources and is handled accordingly but in line with their specific requirements, like cultural needs, equity is essential to the plan (Haahr et al., 2020). Furthermore, the priority on ethical decision-making is emphasized by the patient empowerment, permission, and family engagement aspects. The idea is that it increases public confidence in medical services and gives patients more autonomy (Haahr et al., 2020).

Regarding the possible effects on the plan of particular health care laws, regulations, and clauses, such as the Affordable Care Act (ACA), the HIPPA act, appropriate therapies obtained through primary care Medicaid and other laws. Important aspects of the ACA include cost-effective care, an emphasis on protection, efficient treatments, performance improvement, and simple access to healthcare. These guidelines have an impact on the care plan and work best together since both focus on delivering high-quality care utilizing affordable procedures (Courtemanche et al., 2018).  Patient satisfaction is increased by cost-effective treatment, while caregiver burnout is reduced by a smaller nurse-to-patient ratio. It follows logically that provisions have a favorable impact on the coordination plan (Courtemanche et al., 2018) . Moreover, the implementation of high-quality patient-centered treatment, illness prevention, and system is considered possible through performance improvement measures (Isola & Anil, 2021).

This leads to the conclusion that including provisions for preventative actions lowers illness rates. In order to enhance the results of insurance claims and data protection, the HIPPA act, compensation claims, and fraud prevention policies encourage better recordkeeping, electronic health records, and the security of patient information (Isola & Anil, 2021). Considering that Medicaid does not cover all illnesses, treatment programs, and drugs, one unfavorable consequence may be that it becomes more challenging to deliver high-quality care with a high caregiver ratio at a reasonable rate (Haahr et al., 2020). When everything is taken into account, it is evident that caregivers including you and I play a crucial role in providing health care services. In addition to being caregivers, we also serve as educators, decision-makers, supporters, caretakers, managers, and organizers. As a result, we play a crucial role in the delivery of health care in many ways, and our collaborative efforts have the power to change the way integrated care is provided.

Awareness of Nurse’s Vital Role in Coordination and Continuum of Care

In order to improve medical practitioners’ care effectiveness and outcomes throughout patient populations and care facilities, The American Nurses Association (ANA) acknowledges and openly supports caregivers’ crucial role in the integrated care method. This also ensures the efficient and effective use of health care costs. The nurse practitioner has demonstrated knowledge and innovation in the creation, implementation, and evaluation of efficient team-based care coordination techniques and methods used in cooperation with other healthcare professionals (Haahr et al., 2020). The achievements of nursing personnel undertaking care coordination processes must be evaluated, analyzed, and appreciated to ensure suitable financial and structural advantages for the specialist assistance in the coordinating role.
