 Hello all, my name is Michelle Chavis and I will present today the important issue of care coordination in interdisciplinary settings which remains a vital component of healthcare effectiveness and sustainability in the modern era of technological development.  Solutions and facilities, which is care coordination. This presentation will address all the nursing staff and related stakeholders and address the concept of care coordination that plays a vital role in the improvement of care quality of treatment. Care coordination is believed to associated with several positive outcomes and results such as improving the affordability of healthcare services in America. Patients, healthcare professionals, and family members will all benefit from my presentation. Recently, there has been a surge in the patients commonly suffering from chronic diseases due to COVID-19 and other habitual issues. The benefit of this presentation is for all those patients having complicated medical conditions including hyperglycemia that is associated with blood glucose irregularity and causes diabetes and stress in people. 

Evidence-Based Effective Strategies for Collaborating with Patients and Their Families 

Giving appropriate treatment to these patients is important through team and professional collaboration to achieve efficient outcomes. Many healthcare experts in our organization believe that interprofessional collaboration can help us to reduce the events of misdiagnosis. The important of coloration and continuum of care is increasing day by day; these are some vital components of a good healthcare culture that does not happen by itself. For example, among nurses and physicians, communication is described by the Agency for Healthcare Research and Quality (AHRQ) as a conscious and deliberate effort to improve the acceptability and availability of healthcare quality services for the patients (Young, 2020). Collaboration is deeply associated with healthcare administration and the incorporation of the cutting-edge technology such as electronic health records EHR systems Nurses can enhance the level of coordination by communicating their healthcare expertise with other employees including senior and junior coworkers to form a knowledgeable team. They can strive to ensure smooth healthcare delivery and service across all wards and emergency departments.  Collaboration among several physicians including finding a tailored approach that is able to provide an adaptive treatment to meet the needs of patients. Coordination can empower patients in achieving the quality treatment that is lacking in other organizations not following the guidelines of the American Nurses Association (Young, 2020). For example, according to the guidelines of the ANA, nurses should not allow excessive vests per day per patient in the hospital to avoid any quality issues. The self-management of patients should be encouraged through raising their confidence in treatment. Moreover, the healthcare expenses should also be reduced to increase the rate of survivors not financially sought-after. This means that good coordination allows nurses to get familiarized with national frameworks to improve their skills.  

Culturally Sensitive Strategies for Collaborating with Patients

In this presentation, the culturally sensitive communication techniques are to be implemented in different mechanisms. I will consider my own sociological aspects in the light of cultural frameworks and will analyze norms to improve the honest collaboration between stakeholders. The role of empathetic collaboration is pivotal in healthcare organizations employing diverse employees. The cultural differences are obvious and evident in people who think differently with a range of ideas, behaviors, and traditions (Wittenberg, 2019). For example, nursing staff and practitioners must be able to understand specific health interactions to understand the viewpoint of patients and their families. The assessment of how customers can effectively communicate in a culturally sensitive manner. Every individual has a different attitude based on their beliefs and their culture; which is why effective communication and appreciation for an individual’s traditional attitudes is indispensable to help them engage in honest collaboration. This includes communicating with patients and coworkers clearly by using effective listening techniques and use compassion that helps to create a bond between stakeholders.

Furthermore, it is vital to reduce indirect conversation and replace them with direct interactions. For instance, disputes can be avoided in conversations by limiting the use of mobile applications at workplaces for personal use and only use them for professional consultations.   

Aspects of Change Management

Many recently published pieces of literature believe that organizational change is a crucial aspect of improving positive outcomes related to patient care and quality. Since the organizations are often in the state of struggle to achieve desired outcomes, there are radical changes needed to help employees improve their career paths. The healthcare standards are not met which require embracing organizational change using evidenced-based models and practices. Change management is important for fixing healthcare disparities and procedures. For example, using the model provided by the ACE, nursing staff can easily assess the output and updates the strategic plans and incorporate effective leadership and managerial practices. The right economic ideology must also be incorporated in order to ensure effective change. This requires already existing resources and finances to be utilized on the basis of equity. Another factor that relates to change management in the organization relates to the level of training and education that is necessary for developing the right expertise in nurses and practitioners. This includes regular mentoring of stakeholders to help them understand the culture-based treatments and patient-focused mechanisms (Starbird et al., 2019). Therefore, change management is related to aspects of care that are highly related to patient-centered strategies, nursing coordination patient safety and security, stability, and improving communication with family members.  Moreover, Kim (2019) explains that culturally diverse patients are using different varieties of languages. This is why in order to achieve stronger outcomes; language diversity must also incorporate change strategy. Finally, the patient-to-nurse ration in the organization can play a vital role by raising the satisfaction of customers. The lower this rate or the ration, the greater the positivity and less exhaustion and discrepancies in nurses’ actions. This change management plan will help to reduce mediation errors as well.  

Rationale for Ethical Decision Making in Coordinated Care Plan

The basis of care coordination pan should be on the foundations of moral and ethical decisions-making processes and practices. The most important elements or aspects of incorporating ethical decision-making in coordinated care plans are to ensure that the professionals are practices fairness, equality, justice, obligation, truthfulness, dignity, and accountability. This means that nurses should use and share the resources available and technologies equally across the community. If there is an imbalance of medical resources observed, it highlights the clear sign of a social or a community injustice (Jones, 2019). This means that personal biases and prejudices have no place in an ethical model of operation in the organization that improves the level of fair play among all the stakeholders.

Therefore, making nurses more accountable for their decision and ethical actions can affect patient populations positively to ensure that the ethical principles and standards are adhered to. This means that financing and assets associated with implementing ethical improvement plan are all used honestly. The strategy can help nurses to become ethically efficient when it is perfectly adopted in all departments and can reduce interference and ethical barriers.

Impact of Health Care Policy Provisions on Outcomes and Patient experiences

The potential impact of this particular ethical policy provision on patient care outcomes and experiences will be good. Nurses are better off following the Affordable Care Act (ACA) of 2010 to improve their medical system by introducing change. For instance, the users of health insurance such as Medicaid are facilitated by the friendly policies of the federal government and the state to demonstrate care coordination (De La Pena, 2019). The health insurance market is growing rapidly in the country due to rising poverty issues; most patients are unable to bear the minimum cost of their costly healthcare expenses. As a consequence, the ACA facilitate these families and chronically ill patients to benefit directly from the insurance coverage’s and safeguard their futures. The 2016’s Hospital Corporation of America (HCA) survey shows that many patients who culturally refuse to accept insurance benefits are held responsible for their wellbeing by the ACA. Moreover, the role of Health Insurance Portability and Accountability Act HIPAA is also indispensable to discourage manual record-keeping and encouraging using electronic health records to safeguard patients’ data.

Nurse’s Role in Coordination of Care

Now let me shed some light on the role of nurses for raising awareness regarding coordination and continuum of care. Efficient coordination of health care cannot be possible without the facilitation and support of nurses who play their role in coordinated care efforts. Care continuum happens when nurses play their important role by focusing on the needs and interests of patients. Their role in providing an engaging and memorable presentation to the stakeholders and learns starts with using comprehensive and patient-centered approaches. Nurses can use culturally compatible techniques to use techniques appropriate for audiences to provide an error-free presentation. They can favorably impact the capacity of care coordination in clinical settings by embracing transformational leadership to improve projects’ outcomes and the coordinated delivery of services. Nurses can help senior physicians and decision-makers to formulate effective care coordination guidelines and procedures to enhance patient care outcomes. Their change management skills and strategies can help to improve coordination and mutual trust that will reduce hospitals’ expenses and improve the efficacy of patient-centered coordinated care (Cilicia, 2021).
