Nurs FPX 6200 Assessment 5 Practicum Hours focuses on documenting 50 hours of quality improvement work using SOAR analysis. As part of an active quality improvement initiative to increase patient discharge education, reduce discharge 30-day readmission rates, and improve multidisciplinary team communication in an inpatient hospital, I spent 50 hours learning and providing in-service sessions. The analysis began with a SOAR (Strengths, Opportunities, Aspirations, and Results) analysis to assess how the organization, safety practices, and leadership model are currently structured. This also included being part of interdisciplinary safety huddles, review of discharge procedures, and analysis of patient satisfaction data to see where improvement can be made.
I spoke with a lot of nurse managers, case managers, pharmacists, and IT during the process of learning more about the discharge planning process and EHR integration. I have also seen the teach-back education methods used in the discharge process. I observed the barriers involved, such as limited time and language barriers, and helped develop the plans for improving the staff involvement. I also attended stakeholder meetings and provided data-based outcomes and participated in planning sessions, which are designed to develop culturally competent discharge tools and onboarding procedures for Transitional Care Nurses (TCNs).
Additionally, I took part in developing training on cultural humility and teach-back communication, and the documentation process for staff. I reviewed the patient education resources and assisted in translating the resources to the five most frequently spoken patient languages in the facility. There were also simulation activities to accompany such practice hours, as well as analysis of orientation feedback and reviews based on quality metrics using performance dashboards. The 50 hours were a hands-on immersion to use leadership, communication, and systems-thinking skills in a clinical setting to improve patient outcomes and safety.
